Clipboard and text - screenings (vision, hearing, spinal, acanthosis nigricans)

Industrial ISD follows the Texas Department of State Health Services (DSHS) guidelines in providing school screenings for our students.

Referrals
  • If your child receives a referral for any of the mandated health screenings, this indicates that per our screening guidelines, your child should be evaluated by an appropriate physician or another healthcare provider within a timely manner and the completed referral form turned back into the campus nurse.

  • This will ensure that your child receives any additional support necessary to be successful in and out of the classroom.

  • Please contact any school nurse with questions or concerns.
